The UVM Health Network Home Health & Hospices of Vermont is a non-profit organization with a team of nurses, speech therapists, physical and occupational therapists, licensed nurse assistants, wound care specialists, social workers, home makers and many more. The organization offers a wide range of services to all Vermonters in their very own homes including but not limited to: patient care and specialty programs, family and children's services, tele-monitoring and longitudinal care, long-term care and community services.
