3HC provides Home Health, Hospice and Home Care services across Eastern NC. We are Medicare and Medicaid certified and a 4-star provider. We also accept many private insurances such as BCBS, ChampVA, Tricare and many more. Our Home Health Program includes: Certified Aide Care, Acute and Chronic Disease Management, Medical Social Worker, Medication Management, Occupational Therapy, Physical Therapy, Speech Therapy and Wound Care. Home Care services include complete Personal Home Care Assistance and Concierge Services. For those nearing the end of life's journey our dedicated Hospice Care Teams provide holistic medical care and ensure patients are comfortable. Hospice Services are available at patient/family Home, Assisted Living Facilities, Crystal Coast Hospice House (Newport, NC), Kitty Askins Hospice Center (Goldsboro, NC), and at Skilled Nursing Facilities.
